11.1.1 Format of Petitions

Petitions should be submitted to the Program via Formstack. Petitions will include the specific request, a complete explanation of why the request is being made and supporting documentation. 

Existing petition forms include:

• Withdrawal/Leave of Absence
• Out of sequence course
• Delay of qualifying exam (if not completed by the end of Year 3 Spring term)
• Re-take qualifying exam
• Delay of Prospectus (if not completed by the end of the third year of study)
• Extension/Continuation of the program online beyond 5 years
